Why the old strategies stopped working
Through perimenopause and menopause, falling oestrogen quietly rewrites your biology:
Muscle declines — and with it, the metabolism you used to rely on. Insulin sensitivity shifts — so the same food is handled differently, and more of it heads for your middle. Sleep fragments — and a tired brain is a hungrier, more impulsive brain. Hunger, cravings and recovery all change.
None of this is a character flaw. It's physiology. And it means "eat less, move more, try harder" — the strategy you've been white-knuckling for years — is the old rulebook.
The new rulebook is different: protein and blood sugar stability. Strength. Real recovery. And the mindset work that holds it all together when life gets messy.
I'm a physician, so let me also say this plainly: I am not anti-medicine. I actively recommend women look into HRT with their doctor — the evidence suggests it can help protect your heart and your bones, and I believe women should be offered that conversation far earlier than most are. But a prescription is not a lifestyle, and a ten-minute consultation is not a transformation.

Hello, I'm Dr Orlena
I'm a physician — but that's not why I do this work.
In 2011 I moved to Spain with my eyes wide shut. I hadn't really meant to stop clinical work — that's a long story — but that's what happened, and losing that part of who I was left me quietly churning inside. Then in 2012 I had twins. Four children under four and a half, in a beautiful place, and I remember the moment I saw myself clearly: I was a grumpy mum living in paradise. Exhausted, last on my own list, unsure of who I was anymore — surrounded by sunshine and too worn down to enjoy any of it.
If you've ever looked in the mirror and not quite recognised the woman looking back, you know that feeling.
That was my trigger. And here's what I didn't do: I didn't overhaul my life. I started embarrassingly small — one yoga class a week. That was it. But one kept promise led to another, and another. Today I swim or exercise every single day, and I'm the fittest I've ever been. Not through willpower. Through small habits, kept consistently, until looking after myself became who I am.
Along the way, my doctor's eyes kept seeing the same thing everywhere: intelligent women blaming themselves for a body that had simply changed, handed ten-minute consultations and prescriptions when what they needed was help building a healthy life that actually lasts. Doctors treat disease. Nobody was teaching women how to create health.
So I built Positively Healthy You — to teach what I'd had to live: your body isn't broken, it's changing. You're not who you were — and the healthiest version of you is still ahead, built one small, kept promise at a time.
